Participant Liability Waiver and Release Agreement
Acknowledgment of Risk:
I acknowledge that participation in any physical activities or workshops, including but not limited to Pilates classes, matcha workshops, or events offered by the green flow concept, involves inherent risks. These risks may include, but are not limited to, physical injury, accidents, and health complications.
I confirm that:
1. I am in good physical condition and have no medical condition or injury that would prevent me from participating safely.
2. I will inform the instructor of any health issues, injuries, or limitations prior to participating in any activity.
3. I understand that it is my responsibility to consult with a healthcare professional if I am unsure about my ability to safely participate in activities.
Release of Liability:
In consideration of being permitted to participate in activities or use services provided by the green flow concept, I agree to release, waive, and discharge the green flow concept, its instructors, employees, and partners from any and all liability, claims, or demands arising out of or related to participation, including but not limited to:
• Injuries or accidents during activities.
• Loss, theft, or damage to personal belongings.
• Health complications resulting from participation.
I understand that this waiver does not release liability in cases of proven negligence or willful misconduct as determined by applicable law in Belgium or the European Union.
Agreement to Follow Rules and Safety Instructions:
I agree to:
1. Follow all safety instructions provided by the instructors.
2. Use any equipment or facilities as directed to avoid injury.
3. Respect the guidelines and rules of the studio or event space.
Acknowledgment of Products:
I understand that all products (e.g., matcha, pottery, wellness goods) sold by the green flow concept are to be used as intended, and I accept full responsibility for any issues resulting from misuse or improper storage.
Agreement and Signature:
By signing this form, I acknowledge that I have read, understood, and agree to the terms outlined above. I voluntarily accept all risks associated with participation and release the green flow concept from any liability as specified in this waiver.
